Heat the milk with the caramel simmer and stir until it is melted.

Remove immediately from the heat source and keep stirring until the sauce has cooled down.

Put this mixture together with the ice cream and buttermilk in the blender and mix briefly on the highest level until a creamy thick milkshake.

Pour in high glasses and serve immediately.
