Hip prosthesis surgery is safe on an outpatient basis

  1. Healthy patients with osteoarthritis who are eligible for a hip prosthesis can very well undergo this procedure in day treatment. This is evident from the doctoral research of Yvon den Hartog. Most osteoarthritis patients are still admitted for a few days if they undergo a total hip replacement.

  1. Den Hartog initially investigated how to organize the care around the placement of a hip prosthesis as effectively as possible. For example, patients already practiced walking on crutches before the operation. In addition, attention was paid to good information, pain relief, nutrition, no unnecessary wound drains, urinary catheters and pressure bandages. One on quick start of rehabilitation (get out of bed soon after surgery). "The hospital stay after the procedure was therefore already shortened to such an extent that we thought: wouldn't it even be possible in day treatment," recalls Den Hartog. In April 2014 a pilot was started with an outpatient total hip prosthesis, a European first.

Just as satisfied

  1. Yvon den Hartog followed the orthopedic patients of the day treatment at the Reinier de Graaf hospital for several months. She concluded that they do at least as well as patients admitted a few days after surgery. The patients in the day treatment group were all operated on by orthopedic surgeon Stephan Vehmeijer using the so-called 'anterior approach', a surgical technique in which no muscle attachments are released. Den Hartog: "Patients who are operated with this technique are somewhat more mobile than patients who are operated with other techniques. The anterior approach is more complex from an anatomical point of view. But patients can, because the muscles remain intact. , walk faster without crutches and up a flight of stairs. "

  1. In the Netherlands, more than 25,000 hip prostheses are placed every year. This number is increasing due to the aging population. In order to receive day treatment, patients must be in good health. Cardiovascular diseases and taking blood thinners are taboo. "And of course they have to want it themselves. Also, especially the first night, someone is needed who can take good care of them at home." Follow-up research is still needed. Den Hartog will obtain her doctorate from Erasmus MC on June 15, 2016 for her research.
