Multiple Sclerosis (disease of the central nervous system)

  1. Multiple Sclerosis (MS) is a disease of the central nervous system. The central nervous system consists of the brain and spinal cord. The nervous system is a complex system consisting of bundles of nerves that carry information to and from the brain. These nerves are cells whose cell body is located in the central nervous system.

Possible physiological effects of exercise in MS patients

  1. Sport and exercise have no negative influence on the general course of MS. However, complaints of a temporary nature may arise. The form in which these complaints occur differ greatly per person. MS patients who exercise regularly will find that this has a beneficial effect on balance, stiffness, spasticity, confidence and self-reliance (5). It is also very important for the patient that he learns to explore his own limits through sport and in this way can make optimal use of the possibilities (5).

Objectives for an exercise program for MS patients

  1. As humans we are built to move, and in a healthy person the musculoskeletal system functions without restrictions. But we stay healthy by exercising regularly and by not overloading our body. People with MS have physical limitations and these vary from person to person. For example, one person may be in a wheelchair and can no longer move his legs while the other is still able to do so. It may also be that the movement-related dysfunctions differ per individual, one may experience incontinence problems and the other balance problems.

In summary, these are the goals for MS patients in our exercise program:

Possible risk factors of exercise for the patient group

  1. There are of course always risks when it comes to sports, both for a healthy individual (injuries, fractures), as for an MS patient. But of course there is a big difference in activity and this differs per MS patient. For example, MS patients often have a less confident image of activity due to their limitations (9). This changed self-image increases the barrier to exercise and will first have to be changed in a positive sense. If this does not happen at the beginning of our exercise program, we have the chance that a patient does not feel comfortable during the program and can quickly revert to inactivity. It is therefore wise to do what the patient likes.

The most important risk factors for MS patients who exercise can be:

Movement program

  1. Our exercise program focuses on swimming. Swimming is a good sport for MS patients with paralysis, spasticity and cerebellar symptoms (10). The advantage of moving in the water is that you need less force to exercise and there is less chance of injury. The reduced weight of the body in the water allows for certain movements that the patient may not be able to perform outside of the water. MS patients also often suffer from fatigue and loss of condition. It is important to use your energy sparingly and to choose an endurance sport such as swimming (10).

Measurement methods for evaluating the success of the program

  1. Prior to the program, the patients' heart and lung capacity, condition, muscle strength and degree of mobility will be determined. These will be measured again halfway through and at the end of the intervention.
