What are gluten?
-
For most people eating wheat is actually not a problem and in fact for many people a bagel, a muffin or a nice sandwich is the first thing they grab when they sit down at the breakfast table. But for people with celiac disease, even a little gluten can cause a build-up of painful reactions and those problems can last for days. Gluten exposure is a constant concern for people with celiac disease, but what actually happens when our bodies are exposed to gluten?

-
The symptoms that a person with celiac disease may experience after being â € glutatedâ € ™ may vary, but for the average person it goes something like this:
-
Almost immediately after the gluten has been consumed, the reactions start and this can often be felt by a drop in blood pressure. Shortly afterward, symptoms of reflux may develop, followed by intense fatigue and stomach pain, gas, and bloating, which last for the rest of the day. Insomnia at night is not uncommon and the next day is often characterized by pain and cramps in the intestines and frequent bowel movements, often in the form of watery stools. Moodiness, irritability and anxiety are also among the symptoms, and many people experience problems with thinking, which they describe as "brain fog." An itchy rash and joint pain are also not uncommon. For most people, the symptoms last for two to three days and then go away on their own; all in all, it's quite a hefty price to pay for consuming a tiny piece of gluten.

The main mechanism that causes gluten reactions is gliadin, a protein found in wheat. There are three primary â € typesâ € ™ of gliadin, two of which are associated with celiac disease. The amino acids found in gliadin are the cause of the autoimmune response that accelerates celiac disease and its effects.
-
When a person eats gluten, a complex cascade of events occurs, including the activation of the adaptive immune system, resulting in the production of autoantibodies. This results in the eventual destruction of the villi, the tiny, finger-like protrusions that line the small intestine. Villi are essential for nutrient absorption.
-
Two primary anti-gliadin antibodies are associated with celiac disease: immunoglobulin A (IgA) and immunoglobulin G (IgG). In a healthy immune system, immunoglobulins identify and neutralize foreign bodies such as bacteria, but in an autoimmune disease such as celiac disease, they attack the tissues they would normally protect. In blood tests for celiac disease, IgA is much more common than IgG for good reason: IgA is directly involved in the immune processes of mucosal tissues such as the lining of the small intestine and is also found in abundance in your mucous membranes. When celiac disease is activated, these antibodies attack the villi instead of offering it protection from outside invaders.

Simply put, gluten is a protein found in certain grains that is responsible for those viscoelastic properties that we have come to associate with that spongy bread quality we know and keep it that way. You can further break down gluten into a collection of amino acid chains consisting of Gliadins and Glutenins.
-
Common symptoms of a gluten intolerance such as celiac disease include, but are not limited to: mood-related problems from depression to schizophrenia, ADHD, acid reflux, irritable bowel syndrome, bloating and water retention, seasonal allergies, asthma, sinus congestion, infections, depression, acne, dandruff, seborrhea, psoriasis, heart disease, LDL cholesterol, bladder control problems, dizziness, migraines and headaches, low bone density, colon cancers, diabetes, thyroid problems and stunted growth.
-
Those with celiac disease, which is actually an inherited autoimmune disease and known to be dormant or without symptoms for years, are unable to digest these proteins. When these grains are consumed, they trigger an immune response that causes the body to attack itself and cause damage to the intestinal villi, leading to nutritional deficiencies and malabsorption over time. Celiac disease is very different from gluten intolerance and it is believed that only 1% of the population will be diagnosed with celiac disease.

There are also people who avoid gluten, not because it is trendy, but also because they feel sick after consuming it. Gliadin is the main culprit behind diseases such as leaky gut, inflammation and immune reactions, which we often associate with gluten-related allergies and reactions. However, as with most food sensitivities, the problem is that the symptoms can be varied or delayed, making it very difficult to pinpoint exactly what is causing the common symptoms. You may not get a stomach ache right after eating gluten and IBS may not be a symptom, but constipation is. It can sometimes be a week later that you notice that it made you tired or that it caused complaints that were mood-related. These far-reaching complaints are one of the reasons why there is so much skepticism about whether gluten intolerance really exists.
-
A lack of diagnosis does not mean that someone's symptoms are absent. Those who have successfully identified their food sensitivities and removed the gluten from their diets, thereby experiencing noticeable improvement in their health and symptoms, should not be questioned.
-
Many people, who benefit from a gluten-free diet, often feel best when they consume little to no grains, even gluten-free grains. Grains, like many nuts, seeds and legumes, contain lectins. These are protective molecules in the plant and act as natural pesticides and protect the plant against fungi and animals that want to eat them. They are also very difficult to digest and can irritate the gut wall, leading to leaky gut and making it difficult for nutrients to absorb. It can also cause other food allergies and allergies. Wheat contains high levels of lectins which, despite the cooking process, are still difficult to digest.
-
Another thing that can cause gluten digestion problems is the high processing of grains in general. Today we consume more grains than ever before and this is in the form of highly processed and manipulated pastas, cereals, pastries, etc. Rarely do we eat a bowl of whole grains and they are these starchy and very inflammatory white flour products that can significantly aggravate symptoms.
-
Hidden gluten can also be found in a surprising number of foods. Many people don't realize that these refined grain products are in almost every meal, not to mention household items, cosmetics and beauty products. From salad dressings to vegetable mixes and even alcohol and if you don't know what to look for, it can be hard to avoid. Check out this link for a full list of gluten-related ingredients.
-
A gluten allergy or celiac disease can be more difficult to diagnose than thought. The blood tests sometimes cannot show it clearly. If someone has digestive problems, a test because gut permeability will allow food particles to pass through the intestinal barrier and into the blood, may show sensitivities to many other foods in addition to gluten. Once the gut gets used to this, these sensitivities to other foods can self-dissolve and be re-introduced.
-
In order to diagnose celiac disease, doctors will need to examine the blood for high concentrations of the (tTGA) anti-slip transglutaminase or (EMA) anti-endomysium antibodies. If these test results are negative but the symptoms are still consistent with celiac disease, an intestinal biopsy may be performed to confirm the diagnosis. This is done to check for damage to the intestinal villi. A blood test for IgE and IgG antibodies can also be performed to test for non-celiac gluten allergies.
-
Dermatitis Herpetiformis or DH affects about 15-25% of people with celiac disease. The rash is usually on the elbows, back of the arms, legs and buttocks and can be a good indication of a gluten-related reaction. DH can be confirmed by blood test and skin biopsy.
Self-diagnosis for gluten allergy
-
The diagnosis process can be long, cumbersome and sometimes inconclusive, but there is a simple solution that will provide many benefits. Following a gluten elimination diet can be the best way to see if removing gluten from your diet is really effective.
-
It is best to completely remove gluten for 2-4 weeks in order to clean the system and to notice the improvement. After the elimination period, take in a type of food that contains gluten each time and write down your reaction to this per meal. Has your digestion changed? How was your mood? How long did these effects last?
-
If you noticed significant improvement in your energy, mood and digestion during your experiment, it may be time to adopt a gluten-free lifestyle. Here are some gluten-free recipes to give you inspiration and get you started.

"If a product is not labeled, what are other ways to determine if it contains gluten?"
-
Gluten does not have a specific appearance, color or shape. It is up to the consumer to be well aware of the sources of gluten and the types of foods in which they usually occur. For example, gravy is thickened with wheat flour. Instead of hoping that the gravy was prepared with corn starch, one should really just avoid the gravy, especially if there is no label or cook to confirm this.
-
"Which people benefit the most from a gluten-free diet?"
-
People with celiac disease, a clinically diagnosed condition, have a cell-mediated allergy to gluten, resulting in inflammation of the lower gastrointestinal tract. People with celiac disease should really avoid the consumption of gluten completely. People with gluten sensitivity have not been clinically diagnosed with celiac disease; however, they often show similar symptoms after gluten intake, such as diarrhea, chronic fatigue and headache, which are relieved after the gluten has been excluded from the diet.

"Why is gluten-free so popular?"
-
The past decade has seen greater awareness in the medical profession of the symptoms, advancements in diagnostic techniques and a better understanding in health care of the restorative value of a gluten-free diet in the treatment of these cases, an increased prevalence of both clinically diagnosed celiac disease and documented gluten sensitivity.
-
In addition, there is also a group of consumers who do not have celiac disease or are gluten sensitive, yet swear by the health benefits associated with adhering to a gluten-free diet. However, such physiological effects have not been scientifically validated.
-
"What are gluten-free alternatives to bread, pizza, etc.?"
-
Consumers who want to eat carbohydrates but still avoid gluten can choose from a variety of substitutes, which are formulated and labeled gluten-free. These foods are usually made using one or more gluten-free grains such as corn, quinoa, rice, tapioca, teff, and in some cases oats and are available as bread, cake, muffins, crackers, pasta, and even as Pizza Bottom.
-
In the absence of direct substitutes, consumers who want to avoid gluten can adjust their food choices and replace them with gluten-containing grains. Examples include foods such as rice cakes, rice-based products, tortillas and taco shells made from 100% corn, potatoes of all varieties and sizes, quinoa and quinoa-based foods.

"What is being done to make gluten-free products taste better?"
-
When eating a food, people use many other senses in addition to how something tastes, such as smell (aroma), mouthfeel (texture) and sight (people also eat with their eyes). Due to advances in food technology, sensory evaluation, and packaging, the quality of gluten-free goods has improved significantly over the past five years. Examples include optimizing the beneficial properties of gluten-free grains such as quinoa, teff and rice, de-glutenization of wheat and the use of ingredients that mimic the texture of gluten such as xanthan gum. Other creative approaches include the use of strong yet pleasant flavors such as lemon, chocolate, wild blueberry and cinnamon, to name a few.
